        • Membership plans are not insurance, qualified health plans under the Affordable Care Act, pre-payment for future services, or payment for access to discounted services. Instead, they are direct payment arrangements between plan members and Providers for services, products, and care provided.
        • Membership plans last for 12 months and have an auto renewal feature. Plan members can choose to opt out of the auto renewal at any time through the Member Portal.

        • Payment for treatment not covered in the membership plan is due at the time of service.
        • The savings offered in the membership plan cannot be combined with any other offers or discounts.
